Privacy Policy
Protecting the privacy and confidentiality of information about clients is very important to us. Transition Professionals has policies and practices in place to help ensure that its' employees carry out these practices and otherwise protect personal information about its’ clients. Employees are subject to discipline for violation of these policies.

Collection of Information
As part of Transition Professionals’ normal operating procedures, Transition Professionals and staff acting on its behalf, need to obtain information to determine an individuals’ eligibility for the programs and services it administers, including information required to be collected by funders, researchers and other agencies.

Disclosure of Information
Information directly or indirectly identifying you or any persons currently or formerly receiving services from Transition Professionals is treated as confidential and may only be disclosed as permitted by law and as otherwise described by this Notice.

Neither Transition Professionals nor its staff will use, or share with other parties’, information about Transition Professionals clients for any purpose other than the performance of care by Transition Professionals or to which the client consents.

Neither Transition Professionals nor its staff will further disclose any nonpublic personal information about a former client of Transition Professionals other than as may be required or permitted by law.

Confidentiality and Security of Records
Transition Professionals is governed by HIPAA as well as Part 2 and other State laws which restrict how we may use and disclose your Protected Health Information (PHI). Part 2 and State laws further restrict how we may use and disclose certain drug, alcohol and mental health information. Therefore, generally speaking, we may use or disclose Protected Health Information only when (1) you give us your written authorization on a form that complies with HIPAA, Part 2 and state law; or (2) there is an exception as described in this Notice, such as, but not limited to a court order or medical emergency.

In addition, we are required to obtain your specific written authorization before using and disclosing any personal information which we may maintain except as otherwise permitted by Page 2 law. We must also obtain your specific written authorization for any uses or disclosures we may make for marketing purposes.

You have the right not to give your authorization for a use or disclosure of your Protected Health Information. Except to the extent that we may have taken action in reliance upon a previous authorization we receive from you, you may revoke your authorization at any time by delivering a written revocation statement to Transition Professionals.

Transition Professionals maintains administrative, technical and physical safeguards to ensure the security and confidentiality of its clients’ information and records, to protect against anticipated threats or hazards to such records, and to protect against unauthorized access to or use of such information or records.

Internally, Transition Professionals limits access to its clients’ information to only those staff who need access to the information to perform their job duties. Anyone who misuses information is subject to disciplinary actions. Externally, Transition Professionals does not disclose client information to any third parties unless it has previously informed the client of the disclosure, has been authorized to do so by the client, or is required or permitted to make the disclosure by law or by its regulators.

Emergency Uses and Disclosures
In certain emergency situations, Transition Professionals has the right to exercise its executive discretion to disclose information to emergency medical personnel in order to treat any condition which poses an immediate threat to the health of yourself or any individual, and which requires prompt medical attention. Transition Professionals may only divulge information that is directly pertinent to the emergency at hand. We may also be authorized by law to disclose information in emergencies under other specific circumstances.

Email Information
If you choose to communicate with us via email, we may store the content of your messages together with your email address and our responses. We provide the same protections for these electronic communications that we employ in the maintenance of information obtained via mail or telephone.

Sharing & Selling Information
We do not share, sell, lend or lease any of the information that might uniquely identify a client (such as contact information, mental health records or other personal details) with anyone except to the extent it is necessary to process transactions or provide services that you have requested.

Accessing & Correcting Your Information
You may request access to all of your personal information that we collect and maintain in our database by emailing or faxing us at the usual address or number.

Legally Compelled Disclosure of Information 
We may disclose information when legally compelled to do so. In other words, when we, in good faith, believe that the law requires it or for the protection of our legal rights. We may also disclose personal information when we have reason to believe that disclosing this information is necessary to identify, contact or bring legal action against someone who may be violating our Terms of Service or to protect the safety of our users and the Public.

Other Organizations Linked to Transition Professionals
We are not responsible for the practices employed by organizations linked to, or connected with Transition Professionals, or the information or content contained therein. Often, referrals to other agencies are provided solely to connect clients with information regarding the services that may be useful to them.

Please remember that when you are referred to another organization by Transition Professionals, our Privacy Policy is no longer in effect. Your interactions with any other organization, including those in which we have referred you to, is subject to that organizations own rules and policies. Please read over those rules and policies before proceeding.

How the Information You Provide Us is Used
We use personal information for the following purposes: providing products and services requested by clients, monitoring the use of services, marketing and promotional efforts, improving our content and service offerings, customizing our site’s content, layout, services and for other lawful purposes. These uses improve our organization and services in order to better fit them to meet your needs.

Furthermore, such information may be shared with others on an aggregate basis. Personally identifiable information or business information will not be shared with parties except as required by law.

Occasionally, we may also use the information we collect to notify you about important changes to our programs, new services, and special opportunities we think you will find valuable. You may notify us at any time if you do not wish to receive these offers.

Notice of Information Practices to Clients
Transition Professionals may obtain information about you and any other persons applying for its services. Some of this information will come from you and some may come from other sources. That information and any other subsequent information collected by Transition Professionals may, in some circumstances, be disclosed to third parties without your specific consent as permitted by law. Clients have the right to receive the specific reason(s) for an adverse decision for services in writing. If you wish to have a more detailed explanation of its information practices, please submit a written request to Transition Professionals.
